Consent to Take Part in the NCMH Study


IT IS VERY IMPORTANT THAT YOU READ THE INFORMATION BELOW BEFORE CLICKING THE “I AGREE” BUTTON

If, after reading this information, you would like to take part, you can agree to join the study by clicking on the 'I Agree' button.

Taking part in the National Centre for Mental Health Study is voluntary. If you join and then change your mind, you can leave the study at any time. If you would prefer not to sign up now (for example, because you would like to discuss this with someone else), you can come back to this page later.

The information that you share with us will be held securely by the National Centre for Mental Health research team at Cardiff University.  The study has been given a favourable ethical opinion by Wales Research Ethics Committee (REC) 2 and NHS (Research and Development) permission has also been obtained.

By clicking on the “I Agree” button, you agree to:
 
  1. Provide your contact details (name, postcode, email address, phone number) and some details about your child (e.g. age and gender).
 
  1. Answer some questions about your child and the symptoms they may have experienced via an online assessment - the DAWBA. There will also be an opportunity for your child to provide some information. For NCMH to share the information provided via the DAWBA with your child’s clinical care team.
 
  1. Members of the NCMH research team accessing and using information from your child’s medical records for research.
 
  1. Allow us to link the information you and your child provide to routinely collected, anonymised datasets (such as those held in the Secure Anonymised Information Linkage (SAIL) databank), in order to answer future research questions related to mental health. The data within any such dataset will be fully anonymised and will not be identifiable in any way.
 
  1. Be contacted by the study team who will provide updates about the NCMH research and let you know about other studies that you or your child may want to take part in (for example, via our NCMH newsletters).
 
  1. Be contacted by the study team, to invite you and your child to provide more information about your child’s mental and physical health
 
  1. Allow us to share anonymous data with other researchers.
