Application Form for Young Carers Package (Under 12)

From digital Amazon and Netflix e-vouchers to Headspace mindfulness apps, young carers can now access a special range of opportunities - totally for free! If you’re 11-18 and helping to care long-term for a family member, friend or someone in your community, you're eligible for the Young Scot Young Carers Package. 

As you are under 12, you won't be able to access the package through a Young Scot Membership account, due to data protection reasons. However if you complete this online form, and you are eligible, we will send you two new digital rewards by email. Please name a trusted adult we can send the e-vouchers to, who is able to support you to access.  

Once social distancing measures are reduced, we may be able to send you other offers available in the package via post. 

Remember, on your 12th birthday you can create your Young Scot Membership account at membership.young.scot then re-apply for the Young Carers Package and enjoy a fresh batch of offers! 

Please note for now, you can't apply for a Young Scot National Entitlement Card (NEC). Also, you're not able to order a replacement Young Scot NEC or update your photo. Check the Young Scot card information webpage for updates.

3. To help us make sure the package is useful to all young carers, we would appreciate if you could tell us how your care role feels for you (this will not affect your access to the package). 

Alongside school, socialising and other responsibilities, my care role feels... (please choose one).  *This question is required.
4. Please provide the name and contact details of a trusted adult that would be happy to confirm that you provide a care role. This could be a member of staff at your school or the local carers centre, a youth worker, social worker, nurse, doctor, the person you care for, or anyone else that you trust. 

These details will be used to check that the package is going to young carers and will not be used for any other purpose. If you are unable to provide the details of a trusted adult, please contact membership@young.scot and we will help you with your application. 
5.  Please provide the name and contact details of a trusted adult that would be happy to receive your e-vouchers. This could be the same person who is able to confirm your care role or a member of your household. Your e-vouchers within the package will be sent to them, so choose someone you speak to regularly.
