By completing this form, you are consenting to being contacted by a UNISON representative regarding becoming active in the union. The contact information supplied will only be used for this purpose, and/or to inform winners of the prize draw. Contact data will be deleted at the end of March 2020. Prize winners must be current UNISON members at the time of the draw. Maximum of one prize per member.Any UNISON members who complete this form during November 2019 will be entered into a prize draw and be in with a chance of winning a shopping voucher prize!

For any current UNISON activists, don't worry - you can can get involved too! Any potential new reps that complete this form have the opportunity to say they've been referred by an existing activist. You'll both then be entered into the prize draw. There's no limit to how many times existing reps can be entered, so the more potential reps who say they're referred by you, the greater your chances of winning!
The prize draw will take place on Monday 9 December at 11am. Winners will be contacted using the contact details supplied on this form.
