Members of UNISON Bexley branch can use this form to request assistance from the branch with a workplace issue.
Assistance might include:
- Meeting with you to work out a way forward with addressing your issue
- Speaking 'off the record' with HR or management to discuss your issue
- Accompanying you to a meeting with management and/or HR
- Seeking legal advice from UNISON's solicitors
The assistance we offer will depend on the circumstances you tell us about. We won't raise your issue without speaking to you about it first.
To complete this form, you will need to know:
- Your personal details (name, date of birth, postcode etc.)
- The background to your issue - key dates of important incidents such as discrimination or dismissal are particularly useful
- Some ideas of what you would like to happen which would resolve your issue
Don't forget we can't offer any formal help with any issue that began before you had been a member for four weeks. This is a national UNISON rule which ensures our volunteer union reps can prioritise helping our existing members.
Unfortunately you can't save this form part-way through so you will need to complete and submit it in one session. However if you do run into any issues with completing it please get in touch with us at unison@bexley.gov.uk as it may have saved your work so far.
